The pharmacy price can sit below the list price at the higher doses. The list price is what the NHS reimburses; a private pharmacy charges whatever it chooses, and the pen is identical wherever you buy it. What changes is the total: consultation, delivery and needles are sometimes extra, and a low first month often comes from a one-off code.
Mounjaro Price Increase: Why It Went Up
Eli Lilly raised the UK list price from September 2025, and the NHS reimbursement price was redetermined to match. Per KwikPen, 2.5 mg went from £92 to £133, 5 mg from £92 to £180, 7.5 mg and 10 mg from £107 to £255, and 12.5 mg and 15 mg from £122 to £330 (NHSBSA, Drug Tariff redetermination). The higher doses took the biggest rise, which is why the gap between a starting month and a 15 mg month is wider than it used to be.
If you get Mounjaro on the NHS, none of this reaches you: you pay the standard prescription charge in England, unless you are exempt.
Wegovy Price UK
Wegovy’s NHS list price is lower than Mounjaro’s. The 0.25 mg to 2.4 mg FlexTouch pens each hold four weekly doses; the 7.2 mg dose comes as single-dose pens in a pack of four.

Privately, online pharmacies charge from about £99 for the 0.25 mg pen, about £190 to £230 for 2.4 mg and about £250 to £285 for the 7.2 mg pack, based on standard prices at a sample of UK online pharmacies.
Ozempic is the same medicine as Wegovy, licensed for type 2 diabetes. Its NHS list price is £73.25 per pen for 0.25 mg, 0.5 mg and 1 mg, and £146.50 for 2 mg. Some private prescribers use it off-label for weight loss, but Wegovy is the version licensed for it.
Before You Order
- Compare the price for your current dose, not the starting dose. Most people move up a step every four weeks.
- Check whether the price is the standard one or a first-order code, and whether consultation and delivery are included.
- Check the pharmacy on the GPhC register before you order. A pen sold without a prescription is not a real pharmacy sale.
NHS Eligibility for Weight Loss Injections
Wegovy (semaglutide 2.4mg/week)
NICE approved Wegovy for weight management in TA875. The criterion people most often get wrong is the comorbidity: you need at least one weight-related comorbidity in both BMI tiers, not only in the lower one.
NICE recommends semaglutide only if you have at least 1 weight-related comorbidity and:
- a BMI of at least 35.0 kg/m2, OR
- a BMI of 30.0 to 34.9 kg/m2 and you meet the criteria for referral to specialist weight management services
Plus two conditions on how it is used:
- Within a Specialist Weight Management Service (SWMS) providing multidisciplinary management. You cannot get NHS Wegovy through your GP alone.
- A maximum of 2 years of treatment.
Qualifying comorbidities include type 2 diabetes, hypertension, dyslipidaemia, obstructive sleep apnoea and cardiovascular disease.
If your family background is South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean, NICE recommends using a lower BMI threshold, usually 2.5 kg/m2 lower. In practice that means 32.5 rather than 35, and 27.5 to 32.4 rather than 30 to 34.9. This is easy to miss and it is the difference between qualifying and not.
The rollout has been gradual. Not all NHS trusts have SWMS capacity, and waiting lists can be 3-12 months depending on your area.
Mounjaro (Tirzepatide): Who Qualifies on the NHS Now
Can you get Mounjaro on the NHS for weight loss? Yes. The funded group is much narrower than most people expect. Right now you need a BMI of 35 or above (32.5 with the ethnicity adjustment) plus at least four of five weight-related conditions, and it comes from a participating GP practice, so it depends on where you live. On the NHS you pay the standard prescription charge, not the private price.
Mounjaro is a dual GIP/GLP-1 receptor agonist, targeting two hormones instead of one. Available doses: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg, 15mg.
Its eligibility does not mirror Wegovy’s, and this is where most UK guidance is out of date. There are two different sets of numbers and you need both.
NICE TA1026 recommends tirzepatide for adults with an initial BMI of at least 35 kg/m2 and at least one weight-related comorbidity, with the same 2.5 kg/m2 lower threshold for South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean family backgrounds. On those criteria around 3.4 million people in England qualify.
The NHS cannot fund 3.4 million people at once, so NICE granted a funding variation and NHS England is releasing eligibility in phases over a period of up to 12 years. What you can actually get today is the phase, not the appraisal:

The five qualifying conditions are type 2 diabetes, hypertension, dyslipidaemia, established cardiovascular disease and obstructive sleep apnoea.
Two things follow that are worth knowing before you book an appointment. First, needing four of five conditions is a high bar, and it is why many people who clearly meet the NICE criteria are still told no. Second, unlike Wegovy, tirzepatide is prescribed in primary care: TA1026 names primary care as a point of access, and NHS England funds it through participating GP practices alongside specialist services. Provision is uneven between areas, so whether there is a working pathway where you live is a real variable.
How to get Mounjaro on the NHS:
- Check your BMI and how many of the five conditions you have been diagnosed with.
- Book a GP appointment and ask whether your practice is prescribing tirzepatide for weight management yet.
- If you are in the funded cohort, the GP prescribes it in primary care. If your practice is not taking part, ask how your local NHS area (ICB) is offering it.
- If you don’t qualify yet, private prescribing is the other route.

A 7.2 mg weekly dose has been added as a higher maintenance step. It is not a starting dose: you stay on 2.4 mg for at least four weeks first, and it is intended for adults whose BMI was 30 or above when treatment began. In the UK the 7.2 mg strength is supplied as a single-dose pen, approved by the MHRA on 14 April 2026; before that, the 7.2 mg dose meant three 2.4 mg injections on the same day. Note that this makes 7.2 mg the exception: the UK 0.25 mg to 2.4 mg Wegovy pens are FlexTouch dial-a-dose pens holding four weekly doses each, not one fixed dose per pen. Worth weighing up: altered skin sensation, known as dysaesthesia, was reported by 21.6% of people on 7.2 mg in the STEP UP trial, against 0.3% on placebo. The 2.4 mg figure in the UK SmPC comes from a separate pooled analysis, 2.1% against 1.2% on placebo, so the two are not read against each other. Discuss it with your prescriber before stepping up.

Do not skip titration steps. The most common side effects (nausea, constipation, reduced appetite) are worse when you increase too quickly. If side effects are severe at a new dose, ask your prescriber about staying there longer.
Managing Side Effects
- Nausea - usually worst in the first 24-72 hours after each dose increase. Eating smaller, more frequent meals helps.
- Constipation - increase fibre and water intake. Psyllium husk or a gentle osmotic laxative if needed.
- Reduced appetite - the intended effect, but can be too aggressive initially. Focus on protein-rich foods.
- Fatigue - common in the first 2-4 weeks as your body adjusts to lower caloric intake.
