The Half-Life Difference (and Why It Does Not Matter)

Testosterone cypionate has a half-life of approximately 8 days. Testosterone enanthate is approximately 7 days. In practice, this one-day difference has no meaningful clinical impact. Both esters produce virtually identical blood level curves, and the dosing protocol is the same for both.

Which One Should You Choose?

In most cases, you do not choose. Your provider prescribes what is standard in your region:

  • United States: Testosterone cypionate (200 mg/mL) is the default. Virtually every US TRT clinic prescribes cypionate.
  • United Kingdom: Testosterone enanthate (as Testoviron Depot 250 mg/mL) or Nebido (testosterone undecanoate) for less frequent injections.
  • European Union: Enanthate is standard. Nebido is common for patients preferring injections every 10-14 weeks.
  • Australia: Primoteston (enanthate) is the most prescribed TRT formulation.
  • Canada: Both are available. Cypionate (Depo-Testosterone) is more common.

Can You Switch Between Cypionate and Enanthate?

Yes. They are dose-for-dose equivalent. If you are on 150mg/week of cypionate and switch to enanthate, you take 150mg/week of enanthate. The only adjustment is the injection volume because the concentrations differ:

  • 150mg from 200 mg/mL cypionate = 0.75 mL
  • 150mg from 250 mg/mL enanthate = 0.60 mL

What About Other Testosterone Esters?

Cypionate and enanthate are the two most common esters for TRT, but they are not the only options:

  • Testosterone Propionate: Short half-life (~2 days). Requires daily or every-other-day injections. Rarely used for TRT due to injection burden. Sometimes used for initial dose-finding.
  • Testosterone Undecanoate (Nebido/Aveed): Very long half-life (~33 days). Injected every 10-14 weeks. Convenient but less flexible for dose optimization. Requires in-office injection (Aveed) in the US.
  • Sustanon 250: A blend of four testosterone esters (propionate, phenylpropionate, isocaproate, decanoate). Common in Europe, UK, and Australia. Creates a more complex PK profile with multiple peaks.

TRT in the UK: Sustanon and Nebido

If you are in the UK, the cypionate vs enanthate debate is largely irrelevant. The NHS default for TRT is Sustanon 250, a blend of four testosterone esters (propionate, phenylpropionate, isocaproate, and decanoate) at 250mg/mL. Sustanon creates a more complex pharmacokinetic profile with multiple peaks as each ester releases at a different rate.

Nebido (testosterone undecanoate 1000mg/4mL) is the other common UK option, injected every 10-14 weeks. It is convenient for men who prefer minimal injection frequency, but offers less flexibility for dose optimization.
