Many people use PT-141 alongside a PDE5 inhibitor: PT-141 for desire, Cialis for the physical response. They address completely different parts of the experience.
How to Use It
PT-141 is a small subcutaneous injection, same technique as any other peptide (insulin syringe, abdomen or thigh, the sites on the Vyleesi label). It is not a pill, nasal spray, or topical.
Doses:
- Men (off-label): community use is 1-2mg, starting at 1mg. The label does not cover men; its only dose is 1.75mg.
- Women: community use is 0.75-1.75mg; the labelled Vyleesi dose is 1.75mg.
- First time: Use the lower end of the range regardless of sex.
Timing: the Vyleesi label says at least 45 minutes before anticipated sexual activity. Many users inject 1-2 hours ahead because it takes time to build.
Frequency: As needed, not daily. The Vyleesi label says no more than one dose within 24 hours and no more than 8 doses a month.
Reconstitution (10mg vial, add 2mL bacteriostatic water = 5mg/mL):

What to Know About Side Effects
Nausea is the main one. In the Vyleesi trials 40% of patients had it, usually starting within an hour of the dose and lasting about two hours, and it improved for most people with the second dose. Some users take ondansetron (Zofran) beforehand or start at a lower dose.
Flushing is common and temporary, a warming or redness in the face and chest, usually mild.
Focal hyperpigmentation (skin darkening). The label warns of darkened patches, including on the face, gums and breasts: 1% of patients at up to 8 doses a month, and 38% of patients who took it daily for 8 days. It was more likely with darker skin, and the label says it may not fully clear after stopping, which is one reason it recommends no more than 8 doses a month.
Blood pressure. PT-141 raises blood pressure and lowers heart rate for a few hours after each dose; per the label, both usually return to baseline within 12 hours. Uncontrolled high blood pressure and known cardiovascular disease are contraindications on the label: do not use it if either applies.
