PT-141: The Peptide That Enhances Sexual Desire for Men and Women

Most people have heard of Viagra or Cialis. They work by increasing blood flow, useful for the physical side of sexual function, but they don't do anything about desire. If the issue is that you want to want it more, not just perform better physically, PT-141 is a different answer entirely.

PT-141 (bremelanotide) works in the brain, not the body. It activates the melanocortin system, the part of the brain involved in sexual motivation, and increases desire from the inside out. It works for both men and women, it's FDA-approved in women (as Vyleesi), and as of April 2026 it's available through licensed compounding pharmacies with a prescription.

What PT-141 Actually Feels Like

Users consistently describe a building sense of arousal and desire that starts 45-90 minutes after use and peaks around 2-4 hours. It's not the same as a PDE5 inhibitor (Viagra, Cialis). It doesn't cause an involuntary physical response. It increases how much you actually want sex: the mental and emotional desire, not just the physical mechanics.

Women report this particularly clearly: a genuine shift in interest and motivation, rather than anything forced or artificial. Men report that it works for desire-based low libido where Cialis doesn't address the root issue.

Duration: 6-12 hours for most users.

Onset: 45-90 minutes. Don't re-dose if you feel nothing at 45 minutes. It's still coming.

Who It's For

Men who experience low libido alongside or instead of ED. Cialis and Viagra address the physical response. If the problem is actually wanting to be intimate, PT-141 is what works where they don't.

Women with low sexual desire. PT-141 is literally FDA-approved for this in women (Vyleesi for HSDD, hypoactive sexual desire disorder). It's one of the very few compounds with both formal FDA approval for women and widespread off-label use.

Anyone whose desire has been affected by stress, TRT, or medication side effects. Testosterone therapy, antidepressants, and chronic stress can all dampen libido. PT-141 can help restore that signal regardless of the cause.

Many people use PT-141 alongside a PDE5 inhibitor: PT-141 for desire, Cialis for the physical response. They address completely different parts of the experience.

How to Use It

PT-141 is a small subcutaneous injection, same technique as any other peptide (insulin syringe, abdomen). It is not a pill, nasal spray, or topical.

Doses:

  • Men: 1-2mg. Start with 1mg to assess your response.
  • Women: 0.75-1.75mg. The FDA-approved Vyleesi dose is 1.75mg.
  • First time: Use the lower end of the range regardless of sex.

Timing: 1-2 hours before you want the effects. It takes time to build.

Frequency: As needed, not daily. Most practitioners recommend no more than 2-3x per week.

Reconstitution (10mg vial, add 2mL bacteriostatic water = 5mg/mL):

  • 1mg dose = 0.2mL (20 units on an insulin syringe)
  • 1.5mg dose = 0.3mL (30 units)

Use the peptide reconstitution calculator for your exact volumes.

What to Know About Side Effects

Nausea is the main one, more common above 1.75mg and usually passes within 1-2 hours. Some users take ondansetron (Zofran) beforehand. Starting at a lower dose and titrating up reduces this significantly.

Flushing is common and temporary, a warming or redness in the face and chest, usually mild.

Skin darkening with repeated use over time (same mechanism as Melanotan II), more of a long-term consideration than a session side effect.

Not recommended if you have uncontrolled high blood pressure. PT-141 can cause transient blood pressure increases.

Tracking Your Use

Because PT-141 is used as needed, not on a daily schedule, logging each use alongside how you feel (energy, stress, sleep quality) helps you understand what conditions it works best in and optimize your timing.
