Testosterone replacement therapy is legal and available in Ireland, but protocol quality and waiting times vary enormously between the public system and private clinics. Here is how the Irish system works and your practical options for getting access.

The Two Paths to TRT in Ireland

Path 1: Through the HSE (Health Service Executive)

The HSE is Ireland's public health system. TRT is available through it for diagnosed hypogonadism, though access follows a referral pathway.

The typical process:

  1. See your GP. Request testosterone bloodwork: total testosterone, LH, and FSH, taken as a morning fasting sample
  2. If results show low testosterone with symptoms, your GP can refer you to an endocrinologist or urologist
  3. The specialist confirms the diagnosis of hypogonadism and initiates TRT
  4. Ongoing management may stay with the specialist or return to your GP

Coverage: Testosterone medication is covered by the General Medical Scheme (GMS) for Medical Card holders. For those without a Medical Card, testosterone is available through the community drugs scheme with a small patient contribution, or paid privately at pharmacy prices.

Wait times: Endocrinologist appointments through the HSE can run 6-18 months depending on location. Dublin, Cork, and Galway have shorter waits; rural areas are longer. Urologist waits are sometimes shorter.

The dosing problem: HSE prescriptions often follow conservative dosing intervals: Sustanon 250 every 3 weeks is a common pattern. This produces significant peaks and troughs. Private clinics use weekly or fortnightly injections for much more stable levels.

Path 2: Private Clinics and Telehealth

Private men's health clinics in Ireland offer faster access, optimized protocols, and no long referral waits.

What is available:

  • Private GP appointments or specialist men's health consultations, same week or next week in most cities
  • Testosterone protocols that match international optimization standards (weekly or fortnightly Sustanon, or fortnightly Nebido)
  • Ongoing bloodwork monitoring included in monthly or quarterly fees
  • Telehealth consultations available for follow-up and prescription management

Clinics: Titan Health Ireland, various private GPs in Dublin, Cork, Limerick, and Galway with men's health focus. The private sector for men's hormonal health has grown significantly in Ireland since 2022.

Cost: 150-350 EUR/month depending on provider, which typically includes consultations and monitoring. Medication is purchased separately at pharmacy prices.

Most Commonly Prescribed Testosterone in Ireland

CompoundNotes
Sustanon 250The most commonly prescribed injectable in Ireland. Standard HSE is every 2-3 weeks; weekly is better for stable levels.
Nebido (testosterone undecanoate)Long-acting (every 10-14 weeks). Commonly used in both HSE and private settings for fewer injections.
Testogel 50mgDaily topical gel. Commonly prescribed by GPs; less popular for optimization due to variable absorption.
Testosterone enanthateLess commonly prescribed in Ireland than Sustanon, but available through compounding pharmacies or specialists.

A note on Sustanon: Sustanon contains four testosterone esters with different half-lives. For stable blood levels, weekly injection is significantly better than the standard 3-week interval. Many private clinic patients in Ireland switch to weekly Sustanon 250 (splitting the same vial across four injections) for much better hormonal stability.

Blood Work and Monitoring

Your core panel for TRT in Ireland:

  • Total testosterone (primary measure; request morning fasting sample)
  • Free testosterone and SHBG (often more clinically meaningful)
  • LH and FSH (baseline; typically suppressed once on TRT)
  • Oestradiol (E2) (relevant for symptom management)
  • Haematocrit / haemoglobin (monitor for polycythaemia)
  • PSA (baseline and periodic monitoring for men over 40)

Blood work through the HSE is covered under GMS for Medical Card holders. Private labs (Affidea, Spectra, Medrex) allow direct booking without GP referral for self-paying patients.

Tracking Your Protocol

The TRT dose calculator calculates your exact injection volume for any testosterone concentration. Sustanon 250 is 250mg/mL, so 0.5mL = 125mg, 1mL = 250mg. The split dose calculator helps if you are splitting your Sustanon vial into weekly injections.
