GLP-1 Medications in Sweden: Ozempic, Wegovy, and the TLV Decision (2026)

If you have type 2 diabetes in Sweden, GLP-1 access is actually straightforward. Ozempic is well covered. If you're looking at Wegovy for weight management, the story is different: Sweden's drug assessment agency rejected subsidizing it in early 2026. Here's what that means practically.

The Two-Track GLP-1 Landscape in Sweden

The most useful framing for Sweden's GLP-1 situation is a clear split between two use cases:

Track 1: Type 2 diabetes. Ozempic is covered through Läkemedelsförmåner (the pharmaceutical benefits scheme) for T2D patients who meet prescribing criteria. Your out-of-pocket cost is low. Access is through your GP (primärvård) with no specialist referral needed for most patients.

Track 2: Weight management without T2D. Wegovy is available in Sweden (you can get a prescription). But TLV (Tandvårds- och läkemedelsförmånsverket, Sweden's health technology assessment agency) rejected subsidization for Wegovy in February 2026. The reason: TLV assessed the cost per QALY (quality-adjusted life year) at approximately SEK 720,000, which exceeded their threshold for public subsidy. As a result, Wegovy is not covered. You pay out of pocket.

These two tracks have very different practical experiences. Which one you're on shapes everything about cost and access.

Ozempic in Sweden: How Coverage Works

For type 2 diabetes patients:

Ozempic (semaglutide) is covered under Läkemedelsförmåner when prescribed for T2D. The coverage criteria as of 2026 require that Ozempic is appropriate after considering or trying earlier-line agents, or in situations where those alternatives are not appropriate for the patient (based on comorbidities, tolerability, cardiovascular risk factors).

Your GP at primärvård can prescribe Ozempic for T2D. You do not need a referral to an endocrinologist to start Ozempic for diabetes management in Sweden.

Under the high-cost protection scheme (högkostnadsskyddet), a patient's annual out-of-pocket cost for all subsidized medications combined is capped at approximately SEK 2,600. For most T2D patients on Ozempic, medication cost is well within that cap.

The TLV Wegovy Decision: What It Means

In February 2026, TLV issued its health technology assessment for Wegovy (semaglutide 2.4mg for weight management). The outcome: no subsidy recommendation.

TLV assessed the cost per QALY at approximately SEK 720,000. Swedish healthcare's established threshold for cost-effectiveness is generally considered to be in the range of SEK 500,000 to SEK 600,000 per QALY for serious conditions. Wegovy came in above that threshold.

What this does not mean: Wegovy is not banned in Sweden. It is not unavailable. It is a registered medicine in Sweden, and any GP or specialist can prescribe it for weight management. TLV's decision only affects subsidy.

What it does mean: if you want Wegovy for weight management, you pay out of pocket. No Läkemedelsförmåner coverage. No high-cost cap protection for this medication.

Monthly cost for Wegovy in Sweden without subsidy: approximately SEK 1,200 to SEK 1,600 per month depending on dose and pharmacy.

Saxenda and Other GLP-1 Options

  • Saxenda (liraglutide 3mg): available in Sweden for chronic weight management. Not subsidized for most patients (coverage criteria are narrow). Out-of-pocket cost. Saxenda is a daily injection rather than weekly.
  • Rybelsus (oral semaglutide): available in Sweden for T2D and covered under Läkemedelsförmåner for appropriate diabetes patients. Not a standard weight management option.
  • Mounjaro (tirzepatide): approved in Sweden and being assessed by TLV. As of 2026, TLV has not completed its subsidy review for tirzepatide. For T2D patients, it is available via private prescription. Coverage status may change as the review progresses.

Private Weight Management Clinics

Private weight management clinics in Sweden have grown alongside the GLP-1 category. Clinics in Stockholm, Gothenburg, and Malmö offer consultations for GLP-1 prescribing without the referral requirement.

For Wegovy specifically, the prescription is straightforward (GPs can prescribe directly). The barrier is cost, not access. The private clinic model adds consultation costs but can speed up access versus waiting for a GP appointment in the public system.

Tracking Your Titration

Whether you're on Ozempic for T2D or Wegovy for weight management, the titration schedule is the same principle: start low and increase every 4 weeks based on tolerance. Side effects (nausea, GI discomfort) cluster in the early titration phase and typically improve as you settle into each dose level.

The GLP-1 dose calculator models your titration schedule and helps you track where you are in the ramp-up. Log each weekly dose in Regimen to build a record of side effects, weight response, and energy shifts across the titration curve.

Common Questions About GLP-1 in Sweden

Is Ozempic covered in Sweden?
Yes, for type 2 diabetes. Ozempic is covered under Läkemedelsförmåner for T2D patients who meet prescribing criteria. Under the high-cost protection scheme, annual out-of-pocket costs are capped at approximately SEK 2,600 for all subsidized medications combined.

Is Wegovy covered in Sweden?
No. TLV rejected subsidization for Wegovy in February 2026, citing a cost per QALY above Sweden's standard threshold. Wegovy is available via prescription but costs approximately SEK 1,200 to SEK 1,600 per month out of pocket.

Why did TLV reject Wegovy's subsidy?
TLV assessed the cost per quality-adjusted life year at approximately SEK 720,000, which is above Sweden's standard cost-effectiveness threshold. This assessment is based on clinical trial data and Swedish healthcare pricing benchmarks. TLV decisions can be appealed or re-evaluated as new evidence or pricing changes occur.

Can a Swedish GP prescribe Wegovy?
Yes. Any Swedish GP can prescribe Wegovy for weight management without a specialist referral. The prescription is not restricted; only the subsidy is unavailable. You pay the full cost out of pocket.

Is Mounjaro available in Sweden?
Mounjaro (tirzepatide) is approved in Sweden and available via private prescription. TLV's subsidy review for tirzepatide was ongoing as of early 2026. Coverage status may change as that review concludes.

Tracking Your Protocol

For T2D patients on subsidized Ozempic: injection reminders, dose history, and blood glucose tracking in one place. For weight management patients on Wegovy paying out of pocket: tracking weight trends, titration progress, and side effects helps you document the protocol value over time.

Regimen supports all of these. Available on iOS and Android, with Apple Health and Google Health Connect integration.
